Use of service

Practiq is a clinical documentation co-pilot for Nurse Practitioners (NPs) and Physician Assistants (PAs). By using Practiq, you agree to use the service only for lawful purposes and in compliance with your practice's policies and applicable healthcare regulations. You must hold valid credentials (NP, PA-C, APRN, CRNA, or CNM) to use this service.

Clinical disclaimer

Practiq does not provide medical advice. All generated notes — including SOAP notes, H&Ps, procedure notes, and other documentation — must be reviewed and verified by a licensed clinician before use in any medical record. Practiq is not responsible for clinical errors in documentation.

PHI and HIPAA

Practiq includes a built-in PHI scrubber that automatically detects and removes potential Protected Health Information from your recordings before AI processing. Despite this safeguard, you are ultimately responsible for ensuring no PHI is present in your final documentation. Do not intentionally include patient names, MRNs, dates of birth, or other identifying information in recordings. Practiq is a documentation aid, not an EHR.

Subscriptions

Practiq offers the following plans:

• Free — 10 notes per month, core features
• Pro ($19/mo) — Unlimited notes, all note types, priority processing
• Team ($49/mo) — Everything in Pro, plus team management and shared templates

Pro and Team subscriptions are billed monthly. Cancel anytime through the billing portal. No refunds for partial months.
