Practiq is a clinical documentation co-pilot for Nurse Practitioners (NPs) and Physician Assistants (PAs). By using Practiq, you agree to use the service only for lawful purposes and in compliance with your practice's policies and applicable healthcare regulations. You must hold valid credentials (NP, PA-C, APRN, CRNA, or CNM) to use this service.
Practiq does not provide medical advice. All generated notes — including SOAP notes, H&Ps, procedure notes, and other documentation — must be reviewed and verified by a licensed clinician before use in any medical record. Practiq is not responsible for clinical errors in documentation.
Do not enter any patient-identifying information into Practiq. This includes patient names, medical record numbers (MRNs), dates of birth, social security numbers, insurance IDs, or any other information that could identify a specific patient. Practiq is a clinical documentation co-pilot — it is not an EHR and is not designed to store Protected Health Information (PHI). While Practiq includes automated safeguards to detect and flag potential identifying information, these tools are supplemental. You are solely and exclusively responsible for ensuring that no PHI is submitted through this service. By using Practiq, you agree that all notes entered are de-identified or fictional for documentation practice purposes. Violation of this policy may result in immediate termination of your account.
